Premium StatementEach month, prior to the due date, GROUP PREMIUM STATEMENTS are mailed detailing the total amountpayable for all insured employees.If you are an iServices client, when your premium statement is posted to the site, you will receive ane-mail notification with the subject line, Your bill has been posted to the site. At that time, you can beginreporting employee change information on the site.All premiums must be paid within 31 days of the due date to ensure continuing coverage anduninterrupted claims service.Always pay premium statements AS BILLED. Any employee changes previously reported and not includedon the current monthly GROUP PREMIUM STATEMENT will be reflected on the next statement with theappropriate back charge or credit.For your convenience, a return envelope is enclosed with each month’s GROUP PREMIUM STATEMENT.To provide for accurate and timely premium posting to your account, always return the first page of yourpremium statement along with your premium payment.Note:If your <strong>Plan</strong> includes multiple coverages, you may receive multiple billings each month.
