Experiencia con el Esquema de Quimioterapia Oxaliplatino,<br />
5- Fluorouracilo y Leucovorina en Pacientes con Cáncer<br />
Colorrectal Metastático<br />

R e s u m e n<br />
Estudios fase III demostraron que la combinación de oxaliplatino, 5 fluorouracilo (FU) y leucovorina (LV )<br />
o f rece beneficio en pacientes con cáncer colorrectal metastásico. Nuestro objetivo fue evaluar la respuesta a esta<br />
combinación de quimioterapia en nuestra institución. Entre los años 2000 y 2004 se evaluaron 21 pacientes con cáncer<br />
c o l o r rectal estadío IV que re a l i z a ron al menos 3 ciclos de quimioterapia con oxaliplatino-5FU-LV en el <strong>Hospital</strong> <strong>Privado</strong><br />
– Centro Médico de Córdoba (edad media al diagnóstico: 62,4 años; 14 varones). La mediana de seguimiento fue de<br />
12 meses. El 57% de los pacientes había realizado quimioterapia en adyuvancia con FU-LV previamente. El pro m e d i o<br />
de ciclos fue 4,19 (3 a 6) por paciente. Cuatro pacientes fueron sometidos a resección de las metástasis. La mediana<br />
de tiempo a la pro g resión fue de 6 meses (95% IC 3,07 a 8,9 meses) y la supervivencia global tuvo una mediana de<br />
40 meses (95% IC 19 a 49 meses). Cuando se excluyeron los pacientes que re c i b i e ron cirugía de las metástasis, la<br />
supervivencia disminuyó a 19 meses. Cuando se compararon pacientes que habían recibido quimioterapia adyuvante<br />
p revia con 5 FU-LV con los que no la re c i b i e ron, el HR de muerte fue de 3,32 (IC 95% 0,7 a 15,5). Conclusiones: E n<br />
nuestra serie, la supervivencia global fue comparable a la literatura. Además, constatamos diferencia en términos de<br />
supervivencia a favor de pacientes no expuestos a quimioterapia pre v i a .<br />

Chemotherapy in colorectal cancer<br />
A b s t r a c t<br />
Phase III trials have demonstrated an advantage in survival with oxaliplatin, 5-fluorouracil (5-FU) and leucovorin<br />
( LV) combination in patients with metastatic colorectal cancer. Our objective was to evaluate response and survival by<br />
using this chemotherapy combination in our institution. Between 2000 and 2004 we evaluated 21 patients with colore c t a l<br />
cancer who underwent at least three cycles of chemotherapy with oxaliplatin, 5FU and LV in the <strong>Hospital</strong> <strong>Privado</strong> de<br />
C ó rdoba (mean of age: 62.4 years; 14 male). The median of follow-up was 12 months. The average of cycles was 4.19 (3<br />
to 6) per patient. Surgical resection of the metastases was perf o rmed in four patients. For all patients, the median time to<br />
p ro g ression was 6 months (95% CI 3.07 to 8.9 months) and the median time of overall survival was 40 months (95% CI<br />
19 to 49 months). When patients that underwent metastasectomy were excluded, the overall survival decreased to 19<br />
months. Twelve out of 21 patients (57%) had previously received adjuvant chemotherapy with 5-FU-LV We compare d<br />
patients that had previously received this chemotherapy with patients that not, and the HR of death was 3,32 (95% CI 0.7<br />
to 15.5). Conclusions: In our patients, the survival was similar to previously reported. We highlight the value of surg e r y<br />
in patients with metastatic colorectal cancer. Also, we found a tendency in overall survival in patients that had not been<br />
exposed to chemotherapy, but it was not significant.<br />

I n t r o d u c c i ó n<br />
El cáncer colorrectal es la tercera causa de<br />
muerte por cáncer en el mundo (1). Aproximadamente la<br />
mitad de los pacientes desarrollan enferm e d a d<br />
metastásica (2). Por muchos años, el tratamiento efectivo<br />
para los pacientes con cáncer de colon avanzado estuvo<br />
limitado al 5 fluorouracilo (5-FU), el cual estuvo<br />
