Vingt-et-unièmes Entretiens Odontologiques <strong>de</strong> <strong>Monastir</strong>, 26-27 avril 2013<br />
Pour la restauration <strong>de</strong>s <strong>de</strong>nts antérieures, différentes solutions thérapeutiques sont à notre<br />
disposition en fonction du cas clinique.<br />
Chez un enfant, dont le parodonte est immature, les restaurations composites, facilement<br />
réalisables et réadaptables, trouvent tout leurs intérêts.<br />
Le but <strong>de</strong> ce travail est d'illustrer à travers un cas clinique la restauration du sourire par <strong>de</strong>s<br />
facettes en composite par technique directe, nous soulignerons les étapes clés pour avoir un<br />
résultat le plus prédictible possible, tout en respectant les critères biologiques, mécaniques,<br />
fonctionnels et esthétiques.<br />
M5. Dental treatments in patients un<strong>de</strong>rgoing radiotherapy for head and neck cancer<br />

Background: Radiotherapy-induced damage in the oral mucosa is the result of the <strong>de</strong>leterious<br />
effects of radiation, not only on the oral mucosa itself but also on the skin, adjacent salivary glands,<br />
bone, <strong>de</strong>ntition, and masticatory apparatus.<br />
Aim: To perform <strong>de</strong>ntal treatments and evaluate their efficiency in patients irradiated for head and<br />
neck cancer.<br />
Patients studied: Fifteen patients, 9 men and 6 women, aged from 18 to 66 years old were<br />
referred for restorative and endodontic treatments. Ten patients had been already irradiated and 5<br />
were treated before radiotherapy. All patients were followed between 3 and 8 years.<br />
Results: Four patients suffered from xerostomia, 3 from trismus and one from maxillary diffused<br />
osteonecrosis. Among 60 cavities performed, 45 % were restored with composite resin. Thirty<br />
teeth were treated endodontically. Two patients had osteonecrosis after maxillary <strong>de</strong>ntal extraction<br />
and 2 others had complete <strong>de</strong>ntal extractions. All <strong>de</strong>ntate patients use daily application of fluori<strong>de</strong><br />
gel.<br />
Conclusion: Management of irradiated patients with head and neck cancer is challenging and<br />
require a multidisciplinary team. Conservative treatments should be consi<strong>de</strong>red whenever it is<br />
possible.<br />
M6. L'obturation canalaire par le système B.<br />

Introduction: Le système B, proposé par Buchanan, simplifie le compactage vertical à chaud<br />
proposé par Schil<strong>de</strong>r en restant fidèle aux principes <strong>de</strong> base <strong>de</strong> la technique verticale (choix et<br />
ajustage du maitre cône).<br />
L'objectif <strong>de</strong> ce travail est <strong>de</strong> présenter, à travers <strong>de</strong>s cas cliniques, le protocole opératoire <strong>de</strong><br />
l'obturation canalaire par le système B ainsi que ses particularités par rapport aux autres<br />
techniques.<br />
Observations: Le maître-cône non standardisé ou <strong>de</strong> conicité majorée doit être bien adapté (tests<br />
tactile et radiologique).<br />
La première phase corono-apicale (<strong>de</strong>scente) assure l'obturation du tiers apical en utilisant le<br />
fouloir adapté et le système B.<br />
La <strong>de</strong>uxième phase apico-coronaire (remontée) assure l'obturation <strong>de</strong>s <strong>de</strong>ux tiers coronaires en<br />
utilisant une alternance réchauffement <strong>de</strong> gutta par le heat-carrier et compactage par <strong>de</strong>s fouloirs<br />
verticaux manuels <strong>de</strong> taille croissante.<br />
Discussion: Cette technique assure une excellente distribution tridimensionnelle du matériau<br />
d'obturation surtout lorsqu'elle est associée à une injection <strong>de</strong> gutta.<br />
La phase <strong>de</strong> <strong>de</strong>scente est rapi<strong>de</strong> permettant l'obturation du tiers apical (canal et ramifications).<br />
Conclusion: La technique <strong>de</strong> compactage en vague permet un gain <strong>de</strong> temps considérable. De<br />
plus la température constante du "heat carrier" permet d'obtenir un ramollissement uniforme et sur<br />
toute la longueur du maître cône.<br />
