Diese Diversität eines niedrigen Gradienten gewinnt durch die Schwierigkeiten bei der genauen Bestimmung der Aortenöffnungsfläche noch an Bedeutung. 1, 24-29 Besonders in Fällen mit niedrigem kardialem Auswurf läuft der Untersucher Gefahr, eine zu niedrig bemessene Aortenöffnungsfläche anzunehmen. In diesen Fällen besteht die Chance auf die Existenz einer kontraktilen Reserve, die bei adäquater Therapie einen positiven Einfluss auf die Prognose haben könnte. Eine Fokussierung auf den aortalen Druckgradienten zur Klärung einer Indikation zum Aortenklappenersatz birgt darüber hinaus die Gefahr, einer Fehleinschätzung zu unterliegen. Ein niedriger Gradient muss dann sehr differenziert betrachtet werden, da durchaus Patienten existieren, die trotz eines niedrigen Gradienten von einem Klappenersatz profitieren können. Die Mitralklappenringbewegung ist ein klinisch leicht zu messender Parameter zur Einschätzung der longitudinalen Funktion und erleichtert durch seine prognostische Aussagekraft die Interpretation eines niedrigen aortalen Druckgradienten. 71
