5. Discipl ine Register: A bound ledger. to<br />
be known as the Discipline Register, will<br />
be maintained at a constantly manned post<br />
in the Internal Investi gation Division.<br />
Spaces will be provided in the ledger for<br />
a Discipline Regi ster Numb e r; the name,<br />
rank, badge number and duty aSSignment of<br />
the alleged violator; the date and hour of<br />
receipt of the telephone report; the nature<br />
of the alleged violation; the name.<br />
rank, and badge number of the reporting<br />
officer; the date and hour of receipt of<br />
the first written report; the date the<br />
case was considered by the Department<br />
Disciplinary Board; and the date of final<br />
action by the Superintendent,<br />
6. Custodian of Discipl ine Register will, on<br />
receipt of a report, give to the reporting<br />
officer the Discipline Register Number of<br />
the case, make suitable entries in the<br />
regist.er, and immediately notify the officer<br />
in charge of the Complaint Investigation<br />
section.<br />
7. Security of Discipl ine Register. Owing to<br />
the fact that the names of some innocent<br />
officers will be recorded in it, the Discipline<br />
Register will be suitably safeguarded<br />
and none but the following will be<br />
permitted access to it: the Superintendent,<br />
the Superintendent's personal Aide,<br />
the Commander of the Bureau of Inspectional<br />
Services. the Departmen t In spector<br />
and Deputy Inspectors . the Commander of<br />
the Internal Investigation DiviSion. the<br />
head of the Complaint Inve stigation Section,<br />
and the Department Advocate.<br />
8. Reports to be Submitted<br />
a. All investigation reports and other<br />
documents relating to disciplinary<br />
