5. Discipl ine Register: A bound ledger. to
be known as the Discipline Register, will
be maintained at a constantly manned post
in the Internal Investi gation Division.
Spaces will be provided in the ledger for
a Discipline Regi ster Numb e r; the name,
rank, badge number and duty aSSignment of
the alleged violator; the date and hour of
receipt of the telephone report; the nature
of the alleged violation; the name.
rank, and badge number of the reporting
officer; the date and hour of receipt of
the first written report; the date the
case was considered by the Department
Disciplinary Board; and the date of final
action by the Superintendent,
6. Custodian of Discipl ine Register will, on
receipt of a report, give to the reporting
officer the Discipline Register Number of
the case, make suitable entries in the
regist.er, and immediately notify the officer
in charge of the Complaint Investigation
section.
7. Security of Discipl ine Register. Owing to
the fact that the names of some innocent
officers will be recorded in it, the Discipline
Register will be suitably safeguarded
and none but the following will be
permitted access to it: the Superintendent,
the Superintendent's personal Aide,
the Commander of the Bureau of Inspectional
Services. the Departmen t In spector
and Deputy Inspectors . the Commander of
the Internal Investigation DiviSion. the
head of the Complaint Inve stigation Section,
and the Department Advocate.
8. Reports to be Submitted
a. All investigation reports and other
documents relating to disciplinary
