(ii) The date, type, and length of time of restraint<br />
application;<br />
(iii) The name and position of the person authorizing the<br />
use of the restraint;<br />
(iv) The name of the staff member applying the restraint;<br />
and<br />

411-340-0180 Standards <strong>for</strong> General Business Providers Paid <strong>with</strong><br />
<strong>Support</strong> <strong>Services</strong> Funds<br />
(Amended 7/1/2009)<br />
(1) General business providers providing services to individuals and paid<br />
<strong>with</strong> support services funds must hold any current license appropriate to<br />
function required by the State of Oregon or federal law or regulation<br />
including but not limited to:<br />
(a) A license under ORS 443.015 <strong>for</strong> a <strong>home</strong> health agency;<br />
(b) A license under ORS 443.315 <strong>for</strong> an in-<strong>home</strong> care agency;<br />
(c) A current license and bond as a building contractor as required by<br />
either OAR chapter 812, Construction Contractor's Board or OAR<br />
chapter 808, Landscape Contractors Board, as applicable, <strong>for</strong> a<br />
provider of environmental accessibility adaptations;<br />
(d) Public transportation providers must be regulated according to<br />
established standards and private transportation providers must have<br />
business licenses and drivers licensed to drive in Oregon;<br />
