(f) Incident reports.<br />
(g) Assessments used to determine supports required, preferences,<br />
and resources.<br />
(h) ISP and reviews. If the individual is unable to sign the ISP, the<br />
individual record must document that the individual was in<strong>for</strong>med of<br />
the contents of the ISP and that the individual's agreement to the ISP<br />
was obtained to the extent possible.<br />
(i) Names of those who participated in the development of the ISP. If<br />
the individual was not able to participate in the development of the<br />
ISP, the individual record must document the reason.<br />
(j) Written service agreements. A written service agreement must be<br />
consistent <strong>with</strong> the individual's ISP and must describe at minimum:<br />
(A) Type of service to be provided;<br />
(B) Hours, rates, location of services, and expected outcomes<br />
of services; and<br />
(C) Any specific individual health, safety, and emergency<br />
procedures that may be required, including action to be taken if<br />
an individual is unable to provide <strong>for</strong> the individual's own safety<br />
and is missing while in the community under the service of the<br />
contractor or provider organization.<br />
(k) A written job description <strong>for</strong> all services to be delivered by an<br />
employee of the individual or the individual's legal representative. The<br />
written job description must be consistent <strong>with</strong> the individual's ISP<br />
and must describe at minimum:<br />
(A) Type of service to be provided;<br />
(B) Hours, rates, location, duration of services, and expected<br />
outcomes of services; and<br />
