(C) Any specific individual health, safety, and emergency<br />

procedures that may be required, including action to be taken if<br />

an individual is unable to provide <strong>for</strong> the individual's own safety<br />

and is missing while in the community under the service of the<br />

employee of the individual.<br />

(l) Personal agent correspondence and notes related to resource<br />

development and plan outcomes.<br />

(m) Progress notes. Progress notes must include documentation of<br />

the delivery of service by a personal agent to support each case<br />

service provided. Progress notes must be recorded chronologically<br />

and documented consistent <strong>with</strong> brokerage policies and procedures.<br />

All late entries must be appropriately documented. Progress notes<br />

must at a minimum include:<br />

(A) The month, day, and year the services were rendered and<br />

the month, day, and year the entry was made if different from<br />

the date service was rendered;<br />

(B) The name of the person receiving service;<br />

(C) The name of the brokerage, the person providing the<br />

service (i.e., the personal agent's signature and title), and the<br />

date the entry was recorded and signed;<br />

(D) The specific services provided and actions taken or<br />

planned, if any;<br />

(E) Place of service. Place of service means the name of the<br />

brokerage and where the brokerage is located, including the<br />

address. The place of service may be a standard heading on<br />

each page of the progress notes; and<br />

(F) The names of other participants (including titles and agency<br />

representation, if any) in notes pertaining to meetings <strong>with</strong> or<br />

discussions about the individual.<br />

(n) In<strong>for</strong>mation about individual satisfaction <strong>with</strong> personal supports<br />

and the brokerage services.<br />
