A study of the quality of life and wellbeing of survivors of childhood cancer in<br />
New Zealand<br />
You have been sent this letter and information sheet (attached) because we want to<br />
invite you to take part in a research study that looks at what life is like for young people<br />
aged 12 to 18 years of age who have survived a childhood cancer. You may have been<br />
sent this invitation because you had an illness that was not called a cancer, but needed<br />
the same treatment.<br />
We hope the information sheet will answer most of the questions you may have, but<br />
there is a lot of writing so if you have any questions or concerns, please contact me or<br />
any of the research team – our contact details are on the information sheet. We are<br />
happy to answer your questions.<br />
