is the preferential source of testosterone, <strong>and</strong> it is estimated that 75% of circulat<strong>in</strong>g<br />

testosterone orig<strong>in</strong>ates from the ovary (ma<strong>in</strong>ly through peripheral conversion of<br />

prohormones by liver, fat, <strong>and</strong> sk<strong>in</strong>, but also through direct ovarian secretion). Androstenedione,<br />

largely of ovarian orig<strong>in</strong>, is the only circulat<strong>in</strong>g <strong>and</strong>rogen that is higher <strong>in</strong><br />

premenopausal women than men, yet its <strong>and</strong>rogenic potency is only 10% of testosterone.<br />

DHT is the most potent <strong>and</strong>rogen, although it circulates <strong>in</strong> negligible quantities,<br />

<strong>and</strong> results primarily from the <strong>in</strong>tracellular 5α-reduction of testosterone. In the past,<br />

measurement of 3α-<strong>and</strong>rostanediol glucuronide, a peripheral metabolite of DHT was<br />

used as a circulat<strong>in</strong>g marker of <strong>and</strong>rogen excess <strong>in</strong> the sk<strong>in</strong> (hirsutism <strong>and</strong> acne), but<br />

its cl<strong>in</strong>ical use is negligible.
