INTRODUCTION<br />

Coronary artery disease (CAD) is one of the lead<strong>in</strong>g causes of mortality <strong>in</strong> men <strong>and</strong><br />

women. Age-adjusted morbidity <strong>and</strong> mortality rates from CAD are 2.5-fold to 4.5-fold<br />

higher <strong>in</strong> men than <strong>in</strong> women (1). This male preponderance is remarkably consistent<br />

across 52 countries with hugely divergent rates of CAD mortality <strong>and</strong> lifestyles (2). Sex<br />

hormones can <strong>in</strong>fluence a multitude of factors implicated <strong>in</strong> the pathogenesis of atherosclerosis<br />

<strong>and</strong> coronary artery disease—the traditional view be<strong>in</strong>g that <strong>and</strong>rogens are<br />

harmful <strong>and</strong> estrogens beneficial. However, the gender disparity <strong>in</strong> CAD may <strong>in</strong>volve<br />

diverse mechanisms rang<strong>in</strong>g from <strong>in</strong> utero sex hormone impr<strong>in</strong>t<strong>in</strong>g, gender-specific<br />

behavior, distribution of visceral body fat to vascular <strong>and</strong> myocardial structural/functional<br />

adaptation to ag<strong>in</strong>g, pressure overload, <strong>and</strong> disease (3). As the therapeutic <strong>in</strong>dications<br />

for <strong>and</strong>rogen therapy widen to “non-classical” <strong>in</strong>dications (4), <strong>in</strong>clud<strong>in</strong>g male<br />
