Policy:<br />

Student complaints are submitted and resolved according to departmental procedures.<br />

Purpose:<br />

To ensure that each student has a fair hearing to all complaints and that complaints are addressed<br />

in a timely, efficient manner.<br />

Procedure:<br />

1.0 A student with a complaint submits the complaint verbally to the appropriate<br />

individual.<br />

1.1 Complaints regarding the occupational therapy program are submitted to his/her<br />

instructor or academic advisor.<br />

1.1.1 Complaints <strong>of</strong> this nature include the student’s belief that a policy,<br />

procedure, or practice has been violated, which adversely affects<br />

him/her.<br />

1.2 Complaints regarding a grade in a course are submitted in writing to the instructor <strong>of</strong> the<br />

course.<br />

2.0 Complaints are documented by the receiver, and a written response is returned to<br />

the student within three working days.<br />

1.1 A copy <strong>of</strong> the written response is forwarded to the program director and<br />

maintained in the departmental file.<br />

3.0 If the student is dissatisfied with the initial response he/she submits the complaint<br />

in writing to the program director.<br />

1.1 The complaint is documented by the program director.<br />

1.2 The initial response is reviewed.<br />

1.3 A written response from the program director is forwarded to the student<br />

within three working days.<br />

4.0 If the student remains dissatisfied with the response, he/she may submit a formal<br />

grievance. (See policy OT 503)
