Fieldwork Level II Assignment<br />

Policy: Fieldwork Level II is an assignment within OCCT 865 and OCCT 868.<br />
Successful completion <strong>of</strong> Fieldwork Level II is also a requirement to sit for the NBCOT<br />
practice exam. Following a termination and/or failure <strong>of</strong> a Level II assignment,<br />
determination <strong>of</strong> repeat or continuation <strong>of</strong> fieldwork assignment will be made by the<br />
academic fieldwork coordinator after a validation <strong>of</strong> competency followed by review and<br />
agreement <strong>of</strong> the full faculty.<br />
Purpose: Fieldwork assignments take place at sites with which the university has an<br />
executed affiliation agreement. The purpose <strong>of</strong> validation <strong>of</strong> competency, review and full<br />
faculty agreement is to afford a clear assessment <strong>of</strong> the student’s current safe and pr<strong>of</strong>icient<br />
practice in all aspects <strong>of</strong> occupational therapy practice as required in the affiliation<br />
agreement.<br />
Procedure:<br />
1.0 It is most desirable that the academic fieldwork coordinator be consulted by the<br />
fieldwork site supervisor, fieldwork educator and student so that remediation strategies<br />
can be attempted to promote student success.<br />
2.0 At the time the fieldwork site determines that client safety is at risk and/or ethics, site<br />
policy or illegal violations have occurred, the student is immediately removed from the<br />
site.<br />
3.0 At the time the fieldwork site and the academic fieldwork supervisor determine that<br />
failure is inevitable with no possibility <strong>of</strong> success, the student is immediately removed<br />
from the site.<br />
4.0 The student is directed to meet with the Academic Fieldwork Coordinator (AFWC)<br />
within the next 2 working days, as AFC schedule allows, following secession <strong>of</strong> the<br />
assignment.<br />
5.0 The site is directed to forward all notes <strong>of</strong> student performance and rationale for<br />
dismissal/failure to the Academic Fieldwork Coordinator within 2 working days <strong>of</strong><br />
termination.<br />
6.0 Student must complete the Self-Assessment <strong>of</strong> Your Fieldwork Challenges and<br />
Problems Leading to Termination form and bring this completed form to the scheduled<br />
meeting with the AFWC within 2 working days <strong>of</strong> termination.<br />
7.0 The student’s academic advisor will be consulted and be kept apprised <strong>of</strong> the<br />
termination and invited to attend counseling meetings on campus as schedule permits.
