Policy: All occupational therapy students must obtain and maintain a copy <strong>of</strong> their health<br />

record <strong>of</strong> immunizations and TB test for the duration <strong>of</strong> their active student status in the<br />

<strong>Master</strong> <strong>of</strong> <strong>Occupational</strong> <strong>Therapy</strong> <strong>Program</strong>.<br />

Purpose: In compliance with OSHA regulations and requirements in healthcare facilities, a<br />

health record <strong>of</strong> Immunization and annual testing for TB must be verified.<br />

Procedure:<br />

1.0 A copy <strong>of</strong> your health and immunization record includes:<br />

1.1 Updated immunization and/or titres for rubella, rubeola, mumps, polio, diphtheria,<br />

pertussis, tetanus, and varicella as determined by your medical specialist.<br />

1.2 Hepatitis B immunization or signed declination <strong>of</strong> immunization must be on file<br />

2.0 TB must be tested for annually. The initial test must be a 2 step Mantoux test. Any<br />

exception must be explained in writing by your medical specialist. Pro<strong>of</strong> <strong>of</strong> current<br />

testing (within one year) from an employer, will be accepted.<br />

3.0 The Academic Fieldwork Coordinator will notify all students in writing <strong>of</strong> the health and<br />

immunization record and TB test requirements and the date these verification copies are<br />

due to the OT secretary's <strong>of</strong>fice.<br />

4.0 The OT secretary will log in all records as they are received and will treat all records as<br />

confidential and keep them under lock and key as they are collected.<br />

5.0 It is the student’s responsibility to maintain the original copies <strong>of</strong> all submitted records.<br />

These records will be presented to the fieldwork educator, by the student, on the first day<br />

<strong>of</strong> the fieldworks assignment.<br />

6.0 Students will sign a release <strong>of</strong> information authorizing the GSU MOT Fieldwork<br />

Coordinator to hold copies <strong>of</strong> student health and immunization records in a locked <strong>of</strong>fice<br />

and release them to fieldwork sites as requested.<br />

7.0 Students will not participate in <strong>of</strong>f-campus course and fieldwork assignments if any pieces<br />

<strong>of</strong> these records cannot be verified.<br />

8.0 A particular site may require additional immunizations or verification <strong>of</strong> health. These<br />

site specific requests must be met prior to beginning assignment at the site.<br />

9.0 At the time <strong>of</strong> termination or completion <strong>of</strong> all Level II fieldwork assignments, these<br />

records will be destroyed in keeping with university policy for destroying student records.
