How to Investigate Adherence to Antiretroviral Treatment: An Indicator-Based Approach Each column has a letter above it. We will give directions for each column in turn. Fill in the form for one patient for each row. Words to be spoken are written in bold italics. They will need to be translated into local languages. However all data collectors should agree the language and words so that they can ask the questions in the same way. If the patient is a child, make sure the age is in years and not months. If the child is less than two years old, take the age to the nearest six months (0.5, 1, 1.5, 2). Otherwise, write the age in years. Column C Note gender (write male or female). Column D What is your occupation? The main point of this question is to get an answer to the next question. If the patient is a child who does not attend school yet, his or her occupation can be preschool, if the child is in school, the occupation can be pupil. Housewife may be an occupation depending on culture. Column E With your illness, are you now able to actively continue with your normal activities? Write Y for yes or N for no. If the person is a child, a mother or caregiver knows the appropriate level of activity of a child that age. 54
