Evaluation Form was somewhat useful. However, half (50%) of <strong>the</strong> respondents stated<br />

that <strong>the</strong> form was less than or not useful, and one-fourth (25%) of respondents stated that<br />

<strong>the</strong> form’s usefulness was neutral.<br />
