Log Keeping 5.1 Principles Centre logs shall adhere to the following principles: Activities and events shall be recorded in chronological order. All log entries shall be clear and intelligible. Written (printed) entries shall be in ink. All entries shall be factual and as complete as possible. All entries shall include the identification of the person making the entry. Approved abbreviations etc., shall be used where possible. Dates shall be recorded in dd/mm/yyyy format. Time shall be indicated in 24 hour notation Local Time. Adjustment of clocks shall be recorded. 5.2 Corrections Log entries should only be corrected by the person who made the original entry and initialled. Corrections shall be made with a single line through the entry so that the original entry is still legible. Errors or omissions noted by another person shall be corrected by making a subsequent entry referencing the original entry. 5.3 Centre Log Identification Centre logs shall be identified with the Belfast Port Control Stamp and/or a printed identification showing the name of the Centre and the date. 5.4 Centre Log Entries 5.4.1 Mandatory Entries The following shall be recorded: Date: 18.08.2008 Page: 5-1 Doc: VTS Manual
