Section: 5 Rev No. 02 Title: Log Keeping Issue No. a The signature of any person removing or borrowing the media 5.11 Equipment Operation Record An Operation Record shall be maintained for each piece of electronic equipment. This should be recorded in Port Control Daily Log sheet. Each Equipment Operation Record entry shall include: Workstation identification Date/time of record Name of malfunctioning component Description of the malfunction Your initials The engineering technician‟s initials and time informed Reference number of broadcast notice if applicable Time equipment restored and engineering technician‟s initials Total outage time The initial of the PCO taking over watch. 5.12 Mandatory entries Equipment Operations Record entries shall be made for any shutdown, failure, abnormal operation and change over from main to standby power of: Radio systems Radar AIS Radio Beacons within the PORT CONTROL responsibilities Computers and printers Facsimile machines Console lights and associated console hardware Any other equipment Date: 18.08.2008 Page: 5-4 Doc: VTS Manual
Section: 5 Rev No. 02 Title: Log Keeping Issue No. a 5.13 Centre Log - archiving Material deemed necessary to be retained as a hard copy shall be arranged in a chronological order and securely stored on a daily basis. Each daily package shall be labelled with workstation identification and date. 5.14 Electronic Distribution Systems Copies of all material sent or received at the Port Control Centre over e-mail, fax, or other electronic means shall be retained in their original printed form. Should a message need to be edited a copy of the message shall be made. Electronic logging entries may be backed up on alternative computer storage media. A hard copy of sent/received faxes shall be retained. 5.15 Audio/video recordings Changing Other than a scheduled change, recordings may be removed for the following reasons: Recording equipment failure Maintenance requirement When the Harbour Master deems it necessary. A blank media shall be used to replace the removed media On removal the media shall be labelled.
