5. Log Keeping<br />

5.1 Principles<br />

Centre logs shall adhere to the following principles:<br />

Activities and events shall be recorded in chronological order.<br />

All log entries shall be clear and intelligible.<br />

Written (printed) entries shall be in ink.<br />

All entries shall be factual and as complete as possible.<br />

All entries shall include the identification of the person making the entry.<br />

Approved abbreviations etc., shall be used where possible.<br />

Dates shall be recorded in dd/mm/yyyy format.<br />

Time shall be indicated in 24 hour notation Local Time. Adjustment of clocks<br />

shall be recorded.<br />

5.2 Corrections<br />

Log entries should only be corrected by the person who made the original entry and<br />

initialled.<br />

Corrections shall be made with a single line through the entry so that the original<br />

entry is still legible.<br />

Errors or omissions noted by another person shall be corrected by making a<br />

subsequent entry referencing the original entry.<br />

5.3 Centre Log Identification<br />

Centre logs shall be identified with the <strong>Belfast</strong> Port Control Stamp and/or a printed<br />

identification showing the name of the Centre and the date.<br />

5.4 Centre Log Entries<br />

5.4.1 Mandatory Entries<br />
