Admissions to primary school in 2011 to 2012
Preferences for voluntary-aided (church) schools and
supplementary information forms
Catholic schools
Wigan residents
If you live in Wigan and one of the schools you want your child to go to is a Wigan
voluntary-aided Catholic primary school, you need to fill in the relevant section of the
Wigan Council primary school application form. We will pass on the information to
the school governors. If you do not fill in this part of the form the school will not have
all the information they need to consider your application against their faith criteria
and your child may not be given a place. If you apply online you need to complete
the school’s own supplementary information form and return it to the school. You
can get a copy from our website or from the school.
Some Catholic schools ask you to show proof of baptism. The school will ask to see
this if your child is offered a place. Please do not send any proofs of baptism to your
local authority.
If any of the schools you want is a Catholic school in another local authority please
contact the school to find out if they have any supplementary information forms you
need to fill in.
Other local authority residents
If you live in another local authority and any of the schools you want to go to is a
Wigan voluntary-aided Catholic primary school, you should fill in their supplementary
information form and then return it to them.
Other voluntary-aided schools
If any of the schools you want your child to go to is a voluntary-aided (church) school
other than a Wigan Catholic primary school, you should fill in their supplementary
information form as well as the Wigan Council primary school application form.
If you do not fill in the school’s own form, they will not have all the information they
need to consider your application against their faith criteria and your child may not be
given a place.

from the school;
by phoning the School Places Team on 01942 486037; or
by downloading one from our website.
If you name a voluntary-aided (church) school as one of your preferences and you
do not fill in their supplementary information form, the school will not have all the
information they need to consider your application and your child may not be given a
place.
All filled-in voluntary-aided (church) school supplementary information forms
must be returned to the school by the closing date of 15 January 2011.
