Preferences for voluntary-aided (church) schools and<br />

supplementary information forms<br />

Catholic schools<br />

<strong>Wigan</strong> residents<br />

If you live in <strong>Wigan</strong> and one of the schools you want your child <strong>to</strong> go <strong>to</strong> is a <strong>Wigan</strong><br />

voluntary-aided Catholic primary school, you need <strong>to</strong> fill in the relevant section of the<br />

<strong>Wigan</strong> <strong>Council</strong> primary school application form. We will pass on the information <strong>to</strong><br />

the school governors. If you do not fill in this part of the form the school will not have<br />

all the information they need <strong>to</strong> consider your application against their faith criteria<br />

and your child may not be given a place. If you apply online you need <strong>to</strong> complete<br />

the school’s own supplementary information form and return it <strong>to</strong> the school. You<br />

can get a copy from our website or from the school.<br />

Some Catholic schools ask you <strong>to</strong> show proof of baptism. The school will ask <strong>to</strong> see<br />

this if your child is offered a place. Please do not send any proofs of baptism <strong>to</strong> your<br />

local authority.<br />

If any of the schools you want is a Catholic school in another local authority please<br />

contact the school <strong>to</strong> find out if they have any supplementary information forms you<br />

need <strong>to</strong> fill in.<br />

Other local authority residents<br />

If you live in another local authority and any of the schools you want <strong>to</strong> go <strong>to</strong> is a<br />

<strong>Wigan</strong> voluntary-aided Catholic primary school, you should fill in their supplementary<br />

information form and then return it <strong>to</strong> them.<br />

Other voluntary-aided schools<br />

If any of the schools you want your child <strong>to</strong> go <strong>to</strong> is a voluntary-aided (church) school<br />

other than a <strong>Wigan</strong> Catholic primary school, you should fill in their supplementary<br />

information form as well as the <strong>Wigan</strong> <strong>Council</strong> primary school application form.<br />

If you do not fill in the school’s own form, they will not have all the information they<br />

need <strong>to</strong> consider your application against their faith criteria and your child may not be<br />

given a place.<br />

by phoning the <strong>School</strong> Places Team on 01942 486037; or<br />

by downloading one from our website.<br />

If you name a voluntary-aided (church) school as one of your preferences and you<br />

do not fill in their supplementary information form, the school will not have all the<br />

information they need <strong>to</strong> consider your application and your child may not be given a<br />

place.<br />

All filled-in voluntary-aided (church) school supplementary information forms<br />

must be returned <strong>to</strong> the school by the closing date of 15 January <strong>2011</strong>.
