7.7 Driver=s License and Driver Safety & Insurance Each employee who may be required to perform duties for the agency which includes driving a motor vehicle may be required, on his or her anniversary of date of hire, to produce to his or her immediate supervisor, a current, valid Ohio Driver=s license and proof of current insurance on any vehicles owned, or leased by such employee. Further, and as a supplement to, or in lieu of such requirement, each such employee shall be required, as a condition of receiving reimbursement for mileage, sign a statement on the reimbursement form: 1. That he or she has automobile liability insurance currently in effect for the vehicle or vehicles used for agency purposes; 2. That he or she has not committed any violations such as may cause his or her driver=s license to be revoked or suspended; 3. That each and every motor vehicle used by him or her for agency purposes is in a road-worthy condition. Further, the driving records of such employees may be periodically checked by the Health Commissioner or his designee under procedures established by the Ohio Bureau of Motor Vehicles. Should there be any circumstances such as makes an employee ineligible to hold a valid Ohio Driver=s License, he or she shall immediately report such fact to his or her immediate supervisor. These circumstances include, but are not necessarily limited to, excessive driving violations and failure to acquire and maintain automobile liability insurance. Failure to maintain a valid Ohio Driver=s license, failure to report any circumstance as described above, failure to permit or cooperate in driving record checks, or falsifying the required statement on the mileage reimbursement form may be cause for discipline, up to and including termination of employment. Section 7.8 Safety Belts Ohio law requires that any individual shall wear safety restraint/seat belts while operating or traveling in an automobile. All employees of the agency are required to wear seat belts while driving or occupying a vehicle during agency related travel. Employees are also encouraged to wear seat belts during personal travel. Failure to wear seat belts while driving or occupying a vehicle during agency related travel may be cause for disciplinary action. (Revised 11/11/98) 75
