4.Increased Immunisation:<br />
Increasing immunisation rates continue to be a priority in order to reduce<br />
the incidence of preventable disease in our community. The DHB is working<br />
closely with the PHOs in <strong>Taranaki</strong> to educate and inform the community on the<br />
benefits and importance of immunisation.<br />
5.Better Diabetes and Cardiovascular<br />
Services:<br />
Diabetes and cardiovascular disease are among the main<br />
causes of ill health in <strong>Taranaki</strong>. The DHB supports the<br />
PHOs in ensuring there are services in place to meet<br />
this target. There have been successful initiatives within<br />
Primary Care towards achieving this target.<br />

6.Better Help for Smokers to Quit:<br />
Huge improvements have been made towards this health<br />
target and we are confident these will continue. By the<br />
year end we were meeting the target. An online learning<br />
tool has been made available for staff and a dedicated<br />
smokefree liaison position has been implemented.<br />
