Medicines<br />

Reconciliation<br />

The early stages of a groundbreaking<br />

Medicines Reconciliation Project, which is<br />

set to reduce medication errors, improve<br />

efficiencies and provide safer transcribing<br />

has been well received by local doctors.<br />

The reconciliation programme supported<br />

by technology provides a more reliable<br />

link between the medication patients<br />

are taking when they come into hospital,<br />

what they take during their stay and what<br />

they leave hospital with.<br />

The programme will help hospital and GP<br />

staff all have up to date information.<br />

Work is part of a national pilot with<br />

Counties Manakau, Waitemata and<br />

Southern DHBs and the National<br />

Medication Safety Programme.<br />

Clinical Leadership<br />

Opportunities<br />

Approximately half of all individuals in<br />

leadership roles in the DHB are from a<br />

clinical background.<br />

The Mental <strong>Health</strong> and Addiction Services<br />

operate a partnership approach to<br />

governance and decision making between<br />

service management and clinical leads.<br />

Similar partnership arrangements are<br />

developing in other services supporting<br />

decision making close to the front line.<br />

The DHB involves clinical leaders in the<br />

Midlands Regional Leadership in Practice<br />

and Advanced Leadership programmes.<br />

<strong>Taranaki</strong> DHB also runs a series of internal<br />

upskilling sessions focusing on the<br />

development of clinical leadership skills.
