Confirm with the facility/institution that all needed equipment is in place<br />

and that all required preparations have been made for the audit;<br />

Offer and coordinate educational lectures and tutorials by the audit team to<br />

the facility/institution.<br />

Each member of the audit team is required to:<br />

(f)<br />

Be familiar with the audit principles and procedures as outlined and<br />

referenced in this publication;<br />

Review the information provided by the facility/institution with the<br />

application and that by the auditing body;<br />

Identify if any additional information is required;<br />

Request additional information either from the corresponding contact<br />

person at the facility/institution or from the auditing body;<br />

Familiarize themselves with all the audit schedule report forms — the<br />

report format summary, checklists and worksheets (see Appendices II and<br />

III and Annexes III and IV);<br />

Prepare for educational lectures/tutorials as applicable.<br />

2.4. AUDIT SITE VISIT<br />

The clinical audit site visit will include an entrance briefing, a review and<br />

an exit briefing.<br />

2.4.1. Entrance briefing<br />

A meeting, the entrance briefing, will be held at the commencement of the<br />

audit site visit. The audit team members and all relevant institution and facility<br />

representatives should attend. The entrance briefing should encompass:<br />

(a)<br />

(b)<br />

(c)<br />

(d)<br />

An introduction of the auditors to the facility/institution officials and staff<br />

members;<br />

A presentation by the facility/institution of its organization, governance and<br />

mission, and other background information. This should include<br />

information about the vision of the facility/institution and the perceived<br />

benefits of clinical audit.<br />

A presentation by the audit team leader on the goals of the audit and on the<br />

audit methodology. The principle of respect for the facility/institution and<br />

patient confidentiality will be reinforced.<br />

Confirmation of the audit schedule.<br />
