Review the facility policies and procedures in regard to QA of equipment<br />

(equipment purchase, usage and replacement, inventory, appropriate checks<br />

before usage, QC, maintenance, particularly with respect to safety and<br />

infection control, data protection and backup);<br />

Check the measures for protection and maintenance of data and equipment,<br />

and review documentary evidence of preventive maintenance programme;<br />

Confirm that equipment usage is by trained and authorized personnel;<br />

Examine storage facilities for consumables.<br />

3.2.4.2. Equipment inventory<br />

The audit team should:<br />

Check that all types of the facility’s equipment are recorded in the<br />

equipment inventory, including: imaging equipment/modalities, software<br />

and hardware for digital imaging and teleradiology, auxiliary imaging<br />

equipment such as viewing devices and contrast pumps, phantoms and<br />

dosimetry equipment, medical support equipment such as wheelchairs and<br />

trolleys, medical equipment for resuscitation, anaesthesia, sedation and<br />

monitoring, and administrative equipment such as computers, printers and<br />

software;<br />

Check that sufficient information has been recorded for each piece of<br />

equipment (see Section 3.1.4.2);<br />

Review the inventories for contrast agents and drugs;<br />

Check for control of medical gases;<br />

Check procedures for tracking of implanted medical devices (e.g. stents and<br />

coils);<br />

Check for protection against power supply fluctuations;<br />

Check for adequacy and maintenance of computer IT network, storage and<br />

backup facilities.<br />

3.2.5. Documentation control<br />

The audit team should:<br />

Check the master list of controlled documents and its contents for evidence<br />

of unique identification of documents (e.g. date, version number, page<br />

numbering and total number of pages) and issuing authority;<br />

Check the availability of up to date documents in offices, imaging rooms,<br />

etc.;<br />
