with patient data should have agreed to abide by the facility and institution rules<br />

in regard to confidentiality (see Section 4.1.3.1).<br />

As a measure of how well the service provided meets the expectations and<br />

needs of patients, the facility should actively seek patient feedback. There should<br />

be policies and procedures in place to address complaints from patients. Records<br />

should be maintained of patient complaints, the results of their investigation, and<br />

actions taken to rectify problems identified.<br />

3.1.7. Communication<br />

Good communication is essential for the effective conduct of a diagnostic<br />

radiology service. Communication systems throughout the diagnostic radiology<br />

facility and with related facilities should ensure reliable, unambiguous,<br />

confidential and timely transfer and recording of information.<br />

There should be no gaps in the flow of information among staff members,<br />

regardless of job responsibilities or working hours.<br />

The facility should have its policies and procedures documented, and these<br />

should be communicated to, and be available to, personnel. Personnel should be<br />

aware of the requirement to know and implement these policies and procedures.<br />

3.2. THE AUDIT PROGRAMME<br />

3.2.1. Mission and vision of the diagnostic radiology facility<br />

3.2.1.1. Objectives of the facility<br />

The audit team should:<br />

Review the mission statement of the facility in the quality manual;<br />

Check the statement in regard to the nature and range of the service<br />

objectives;<br />

Check the facility’s relationship with other specialties and related services;<br />

Discuss with senior management the role of the facility in teaching and<br />

research;<br />

Discuss with senior management the financial structure in place to support<br />

these objectives;<br />

Discuss with senior management the extent to which objectives are being<br />

met, and any impediments to achieving them;<br />
