Pain sense<br />

Superficial pain is tested with a pin but distinction should be made between pain<br />

sensation and fine touch. Pressure pain is tested by pressing hard or squeezing muscles.<br />

Temperature sense<br />

Temperature sense is tested with test tubes containing hot and cold waters.<br />

Adaptation of these receptors can be demonstrated by feeling water at room temperature<br />

after keeping the finger in hot or cold water for two minutes.<br />

Exercise: Describe the change in sensation in various conditions- sensory nerve lesions,<br />

peripheral neuropathy, spinal cord lesions.<br />

Motor System<br />

Bulk of muscles<br />

Observe the muscle bellies and compare with opposite side, look for atrophy,<br />

hypertrophy, tremors and fasciculations.<br />

Tone of the muscles<br />

When the patient is completely relaxed move the limbs gently and feel the<br />

resistance to movement. Look for hypertonia or hypotonia.<br />

Strength of Muscles<br />

Ask the patient to force his limbs against resistance offered by you. Test the<br />

strength of each muscle separately and compare with the opposite side.<br />
