(4) Under Hospital<strong>is</strong>ation Identify Card (HIC), both employees andeligible dependants must request a letter of undertaking from HR.Ward upgrading under CCS <strong>is</strong> applicable for restructuredGovernment hospitals only.39. FLEXI-BENEFITS(1) The Flexi-Benefits Scheme (FBS) <strong>is</strong> an added gesture <strong>to</strong> providegreater flexibility in staff welfare benefits <strong>to</strong> meet individualneeds. With FBS, staff will be empowered <strong>to</strong> use <strong>the</strong> FBSpayment for <strong>the</strong>ir holiday, dental, subscription <strong>to</strong> broadbandinternet at home and o<strong>the</strong>r benefits that best meet <strong>the</strong>ir personalas well as family needs.(2) Under <strong>the</strong> FBS, all staff members who are in service as at 31stMarch each year will be eligible for <strong>the</strong> FBS payment. Thepayout will be a flat rate of S$350 for eligible staff irrespective of<strong>the</strong>ir div<strong>is</strong>ional status and schemes of service. Th<strong>is</strong> payment willbe subject <strong>to</strong> both Employee and Employer CPF contributions(where applicable) and it <strong>is</strong> a taxable benefit. Staff memberswho have already left <strong>the</strong> service on resignation or expiry ofcontract before 31st March will not be eligible for <strong>the</strong> payment.Those who retire from service during <strong>the</strong> year will be given <strong>the</strong>pro-rated amount. There will be no recovery of <strong>the</strong> payout if astaff resigns from service after receiving <strong>the</strong> payment.40. WELFARE BENEFITSThe Polytechnic shall extend <strong>to</strong> all employees <strong>the</strong> following benefits -(a) A baby gift hamper worth $60/- upon birth of a child;(b) A $100/- marriage gift <strong>is</strong> credited <strong>to</strong> <strong>the</strong> employee‟s bank accoun<strong>to</strong>n <strong>the</strong> occasion of <strong>the</strong> employee‟s legal marriage;(c) A fruit / flower basket provided when an employee <strong>is</strong> hospital<strong>is</strong>edandCA 75 of 2011 Page 19 of 61Ngee Ann Polytechnic Academic Staff Agreement, 2011ASCA( NC 290711)
