NGEE ANN POLYTECHNIC ACADEMIC STAFF <strong>AGREEMENT</strong>, 2011GROUP TERMS LIFE POLICYSchedule V(Clause 17)The Group Term Life Policy, which <strong>is</strong> admin<strong>is</strong>tered by Aviva Ltd, covers death and <strong>to</strong>talpermanent d<strong>is</strong>ability. Th<strong>is</strong> <strong>is</strong> a voluntary scheme and <strong>is</strong> open <strong>to</strong> all employees (including parttimers)up <strong>to</strong> a maximum age of 65. The sum assured <strong>is</strong> equivalent <strong>to</strong> 12 times <strong>the</strong> monthlybasic salary. Spouse and children are not eligible <strong>to</strong> apply for th<strong>is</strong> policy. A circular stating<strong>the</strong> age-related premiums will be sent <strong>to</strong> all employees before <strong>the</strong> renewal date <strong>to</strong> inviteinterested employee <strong>to</strong> enrol for <strong>the</strong> policy. New applications will not be accepted after <strong>the</strong>closing date.The <strong>to</strong>tal premium <strong>is</strong> payable equally between <strong>the</strong> Polytechnic and <strong>the</strong> insured employee. Anyextra health premium loading due <strong>to</strong> ex<strong>is</strong>ting illness imposed by <strong>the</strong> insurer will be borne by<strong>the</strong> employee. The age-related premium rate (subject <strong>to</strong> change) payable <strong>is</strong> -Age band Annual Rate per $1,000
