The Halton Geriatric Mental Health Outreach Program (HGMHOP), a satellite program of St.<br />
Joseph's Healthcare Hamilton Regional Geriatric Psychiatry Service, is a unique and innovative<br />
program dedicated to seniors with late onset mental health problems, addictions and/or<br />
behavioural disturbances and their caregivers. Serving adults 65* years of age and over residing<br />
in the region of Halton living in their own homes, retirement or supportive housing and Long<br />
Term Care Homes (*age range is flexible for adults with early onset dementia). They provide<br />
clinical outreach services to those seniors who cannot or will not seek traditional service e.g.<br />
going to an office.<br />
