decentralization, and <strong>in</strong>tegration of a few of the vertical programmes. Some<br />
of these had become veritable <strong>in</strong>stitutions, benefit<strong>in</strong>g from significant<br />
political support. A failure to reorganize central structures to fit new tasks<br />
occasionally led to perpetuation of the exist<strong>in</strong>g system, to the detriment of<br />
change.<br />
Certa<strong>in</strong> communities (e.g., municipalities and local development<br />
committees) had amply demonstrated their commitment by construct<strong>in</strong>g<br />
health centres and pay<strong>in</strong>g salaries for support staff. They often allocated<br />
more than 7% of their development tax to the health <strong>sector</strong>. The<br />
community already met 75% of health expenditures. 12 The ComHC trials,<br />
and the cost-recovery system under way <strong>in</strong> certa<strong>in</strong> localities, demonstrated<br />
people’s will<strong>in</strong>gness to pay for quality services. However, apart from areas<br />
that supported organization projects, such as village associations or the Ton,<br />
management capacities were weak.<br />
EXTERNAL PARTNERS<br />
WHO supported the efforts of national health professionals dur<strong>in</strong>g all the<br />
stages of the 1980s, <strong>in</strong> particular those aim<strong>in</strong>g to decentralize and <strong>in</strong>tegrate<br />
programme management. WHO also supported vertical programmes,<br />
<strong>in</strong>clud<strong>in</strong>g those undertaken on a large scale at the end of the 1980s, for<br />
example, EPI and MCH. At the decisive stages its role was less visible,<br />
though that did not imply distanc<strong>in</strong>g.<br />
UNICEF developed one of the largest support programmes for the<br />
health <strong>sector</strong>. It played a dynamic role at all stages of development, and<br />
amended the 1988-1992 cooperation programme to provide f<strong>in</strong>anc<strong>in</strong>g under<br />
the Bamako Initiative. UNICEF <strong>in</strong>tegrated and later encouraged the process<br />
of cof<strong>in</strong>anc<strong>in</strong>g the national programme with other partners and participated<br />
<strong>in</strong> a jo<strong>in</strong>t evaluation mission with the World Bank to launch a new national<br />
programme (the HPRWP) <strong>in</strong> May 1990. Technical assistance was offered to<br />
the regional offices. While support of the organization towards government<br />
efforts for def<strong>in</strong><strong>in</strong>g a national policy was adequate, it should be noted that<br />
the <strong>in</strong>tensity and vigour applied to re<strong>in</strong>forc<strong>in</strong>g the EPI, a vertical programme <strong>in</strong><br />
itself, was not <strong>in</strong> harmony with the problems and local priorities of the time.<br />
Between 1984 and 1988, the World Bank-supported HDP, <strong>in</strong> Kayes<br />
Region, focused on three districts. The HDP approach had been that of a<br />
