ComHCs. 32 It was an example where actual use built confidence <strong>in</strong> the<br />

methods without clearly resolv<strong>in</strong>g the underly<strong>in</strong>g problems (evolution and<br />

transition <strong>in</strong> the management of certa<strong>in</strong> vertical programmes), which<br />

became <strong>in</strong>creas<strong>in</strong>gly apparent.<br />

A TRAINING STRATEGY<br />

Between <strong>1989</strong> and 1992 the Family <strong>Health</strong> Division <strong>in</strong>troduced new<br />

tra<strong>in</strong><strong>in</strong>g strategies, under a programme that reorganized services for MCH<br />

activities. The reorganization proceeded from health centre to health centre<br />

with on-the-spot facilitators, <strong>in</strong> a process that led to the desired change.<br />

The limitation of this <strong>in</strong>tervention was that it only concerned MCH, even<br />

though the health centres had other needs (e.g., essential drugs, effective<br />

curative care, f<strong>in</strong>ancial and material management, and community<br />

participation). Tra<strong>in</strong><strong>in</strong>g could not be offered by the specialized service of the<br />

central level but required staff or a team <strong>in</strong> the field with more flexible<br />

responsibility and abilities.<br />

Whatever its shortcom<strong>in</strong>gs, the programme proved the basic utility of<br />

on-site tra<strong>in</strong><strong>in</strong>g. In the <strong>in</strong>terest of cont<strong>in</strong>uation, the staff conduct<strong>in</strong>g the<br />

tra<strong>in</strong><strong>in</strong>g were those <strong>in</strong> charge of supervis<strong>in</strong>g the centre. The district team,<br />

<strong>in</strong>clud<strong>in</strong>g the chief doctor, became <strong>in</strong>dispensable for the tra<strong>in</strong><strong>in</strong>g activities<br />

associated with the ComHC. The region provided support to the district<br />

teams to <strong>in</strong>itiate the process and to develop expertise. They also specified<br />

suitable standards and procedures, developed tra<strong>in</strong><strong>in</strong>g modules,<br />

strengthened regional managers’ teach<strong>in</strong>g skills, and evaluated the results of<br />

tra<strong>in</strong><strong>in</strong>g.<br />

The HPRWP Inter<strong>in</strong>stitutional Group for Tra<strong>in</strong><strong>in</strong>g, orig<strong>in</strong>ally<br />

traditional <strong>in</strong> its approach to tra<strong>in</strong><strong>in</strong>g tra<strong>in</strong>ers, saw its role evolve <strong>in</strong> the<br />

context of these new responsibilities. A problem to resolve was how to tailor<br />

the tra<strong>in</strong><strong>in</strong>g programme to the first ComHCs. It was also important to adapt<br />

or create the appropriate medium (e.g., <strong>in</strong>tegrated questionnaire for mother<br />

and child follow-up, schedule, outl<strong>in</strong>e for microplan development,<br />
