1990-1992: towards a strategy for change<br />

Mobiliz<strong>in</strong>g under unfavourable conditions<br />

Political <strong>in</strong>stability <strong>in</strong> the country prevented dissem<strong>in</strong>ation of the new<br />

health <strong>sector</strong> policy at the time desired, and it ended up be<strong>in</strong>g treated as just<br />

another new project. The entire vision for health system <strong>reform</strong> became<br />

marg<strong>in</strong>alized. The practical implications for schedul<strong>in</strong>g activities and<br />

implement<strong>in</strong>g them <strong>in</strong> each region, and for the partners who supported the<br />

plan, were not clear, and were apt to vary accord<strong>in</strong>g to the negotiator at the<br />

central level. The persistent disorder expla<strong>in</strong>s the lessened <strong>in</strong>terest of some<br />

bilateral partners and NGOs. For its part, the community was not <strong>in</strong>formed<br />

of the opportunities that the new policy offered, or the responsibilities it<br />

entailed.<br />

ELIGIBILITY REQUIREMENTS FOR THE DISTRICTS<br />

Some of the regional districts needed to meet the eligibility criteria given <strong>in</strong><br />

Table 1 to receive support for programmes. The first priority was to prepare<br />

district social-health development plans and establish the first ComHC.<br />

Because the f<strong>in</strong>anc<strong>in</strong>g anticipated under the HPRWP did not take <strong>in</strong>to<br />

account the preparatory stages, funds had to be sought from programmes<br />

already under way or those near<strong>in</strong>g completion. Yet programme managers<br />

(national or partners) were seldom <strong>in</strong>cl<strong>in</strong>ed to adjust their support. The<br />

situation of blockage thus created was very frustrat<strong>in</strong>g for national<br />

managers, particularly because actual conditions were far removed from the<br />

criteria detailed <strong>in</strong> Table 1.<br />

A district team with at least two doctors. The State was committed to<br />

endow<strong>in</strong>g all the districts with the required personnel, but very few of the<br />

districts had two doctors available. The structural adjustment programme,<br />

together with the end of recruitment for public service, did not improve the<br />

situation. Only much later, follow<strong>in</strong>g the devaluation of the FCFA <strong>in</strong><br />

January 1994, did the government authorize recruitment to compensate for<br />

the shortage of personnel <strong>in</strong> the districts. Redeployment of exist<strong>in</strong>g<br />

personnel to poorly served districts had limited impact.<br />

A plan for extend<strong>in</strong>g coverage. The method used to prepare plans had<br />

no provision for extend<strong>in</strong>g coverage. It was naturally oriented towards<br />

programm<strong>in</strong>g and budget<strong>in</strong>g available resources for national programmes,<br />
