In the <strong>in</strong>terest of common sense, it was decided that the duration of<br />
<strong>in</strong>itial tra<strong>in</strong><strong>in</strong>g should not exceed one month, to avoid <strong>in</strong>terfer<strong>in</strong>g with plans<br />
for extend<strong>in</strong>g coverage. A prelim<strong>in</strong>ary list of tra<strong>in</strong><strong>in</strong>g fellows was <strong>in</strong>spired by<br />
Gu<strong>in</strong>ean 33 and Tunisian 34 experiences. In addition to organiz<strong>in</strong>g effective<br />
curative care and major prevention activities, and prescrib<strong>in</strong>g essential<br />
drugs, tra<strong>in</strong><strong>in</strong>g emphasized<br />
- healthy <strong>in</strong>fant care as an <strong>in</strong>tegrated activity <strong>in</strong> itself (nutritional follow-up,<br />
vacc<strong>in</strong>ation, breastfeed<strong>in</strong>g promotion, vitam<strong>in</strong> A supplementation, child spac<strong>in</strong>g and<br />
promotion of family plann<strong>in</strong>g)<br />
- us<strong>in</strong>g operational questionnaires to support tra<strong>in</strong><strong>in</strong>g and for develop<strong>in</strong>g <strong>in</strong>structions<br />
for pr<strong>in</strong>cipal activities<br />
- organiz<strong>in</strong>g and manag<strong>in</strong>g the human, material, and f<strong>in</strong>ancial resources of the centre<br />
- microplann<strong>in</strong>g activities, <strong>in</strong> particular, it<strong>in</strong>erant activities <strong>in</strong> the villages and<br />
develop<strong>in</strong>g and <strong>in</strong>terpret<strong>in</strong>g management control data and monthly reports.<br />
Tak<strong>in</strong>g <strong>in</strong>to account the <strong>in</strong>sufficiency or <strong>in</strong>adaptability of some of the<br />
modules and tools from vertical programmes, the National <strong>Health</strong> Office<br />
gave the green light for the regions to test new formulas <strong>in</strong> the ComHCs.<br />
This freedom was helpful <strong>in</strong> implement<strong>in</strong>g the <strong>in</strong>itial ComHCs. Regional<br />
and district staff felt they had a stake <strong>in</strong> the tools they produced and the<br />
modules they adapted. Ultimately, experiences could be exchanged <strong>in</strong> a way<br />
that allowed the central level to exercise its role as harmonizer. The<br />
autonomy carried two risks: first, centrifugal tendencies—the variability<br />
among regions was troubl<strong>in</strong>g to a centre uncerta<strong>in</strong> of its new role; second,<br />
the loss of time and money, lead<strong>in</strong>g to the frequent re<strong>in</strong>vention of the<br />
wheel. The question was one of optimiz<strong>in</strong>g the price to pay for staff<br />
commitment and sense of <strong>in</strong>vestment <strong>in</strong> the programme.<br />
Practical tra<strong>in</strong><strong>in</strong>g was the pr<strong>in</strong>cipal method used <strong>in</strong> build<strong>in</strong>g ComHC<br />
teams. It began with develop<strong>in</strong>g the plan and cont<strong>in</strong>ued up to supervis<strong>in</strong>g<br />
the first centre. Strengthened regional office technical support capacities,<br />
support and enthusiasm at the central level, and <strong>in</strong>creased exchanges<br />
between districts and regions made this approach possible. 35<br />
