difficult to handle. The obligation for the ComHAs to sign an agreement<br />
with the M<strong>in</strong>istry of <strong>Health</strong> was considered by some to be very restrictive.<br />
The response of staff from the M<strong>in</strong>istry was that by the agreement, the<br />
m<strong>in</strong>istry committed itself to a lead<strong>in</strong>g role <strong>in</strong> public health, especially s<strong>in</strong>ce<br />
it was no longer directly <strong>in</strong> charge of first level health structures. The<br />
publication of regulations nevertheless unblocked the situation and helped<br />
to relaunch the implementation <strong>in</strong> the regions and districts.<br />
The viability of the ComHCs rema<strong>in</strong>ed a concern <strong>in</strong> all respects: with<br />
regard to the structure of responsibilities (doctor or nurse employed by the<br />
ComHA) and with regard to the context (urban, rural, rural with lowdensity<br />
population). Help<strong>in</strong>g the ComHAs to assume their new<br />
responsibilities rema<strong>in</strong>ed a challenge, particularly <strong>in</strong> rural sett<strong>in</strong>gs. 69<br />
