This Doctoral study conforms to the National Statement on Ethical Conduct in<br />

Research Involving Humans and has been approved by the <strong>Curtin</strong> <strong>University</strong> Human<br />

Research Ethics Committee.<br />

CONFIDENTIALITY AND SECURITY:<br />

1. All completed surveys will require the consent of the participant;<br />

2. The participant will have the right to withdraw their contribution at any time<br />

up to the publication of the thesis;<br />

3. To ensure that any potential misunderstandings of the subject material are<br />

resolved prior to the interview taking place, the participant will be required<br />

to acknowledge this information sheet and complete the consent form before<br />

proceeding;<br />

4. All participant identities will be kept completely confidential; and<br />

5. All questions are designed in a way to eliminate bias.<br />

RISKS:<br />

Although some questions may be of a personal nature, there are no obvious safety<br />

issues that need to addressed during the participation in this survey.<br />
