The NIHDI publishes yearly the numbers of registered nurses and midwives who<br />

received an id<strong>en</strong>tification number from NIHDI.<br />

In the last 10 to 15 years, self-employm<strong>en</strong>t became more attractive, financially as well as<br />

organisationally 2 . Explanations for higher attraction of self-employm<strong>en</strong>t in home nursing<br />

might be that there is no hierarchical relationship with a supervisor and by visiting more<br />

pati<strong>en</strong>ts and delivering more nursing care, self-employed nurses have the possibility to<br />

acquire a higher income. The personnel cost structure is differ<strong>en</strong>t betwe<strong>en</strong> selfemployed<br />

and employee-nurses. As a result, the proportion of the exp<strong>en</strong>ditures for<br />

home nursing sp<strong>en</strong>t in employee-nurses decreased from approximately 60% in the<br />

1990s to approximately 40% of all NIHDI-exp<strong>en</strong>ditures in home nursing in 2005 2 .<br />

2.4.2 Qualification levels<br />

Nurses who work in home nursing need to have one of both nursing qualifications that<br />

are recognized by the directive 2005/36/EC and have to apply for a NIHDI-id<strong>en</strong>tification<br />

number. In Belgium, two levels of nurses (bachelor-level (A1) and diploma-level (A2))<br />

comply to the EC directive.<br />

As with other health care professionals, the auth<strong>en</strong>ticity of nursing diplomas is verified<br />

by provincial medical committees of the Federal Public Service Public Health, Food<br />

Chain Safety and Environm<strong>en</strong>t. Anyone who is not properly registered is not allowed to<br />

practise. The lic<strong>en</strong>ce is giv<strong>en</strong> for an unlimited time – that is, once health care<br />

professionals have be<strong>en</strong> giv<strong>en</strong> the right to practise, they do not have to prove<br />

continuing professional education. However, in cases of malpractice, lic<strong>en</strong>ces can be<br />

withdrawn.<br />

Five qualification levels have be<strong>en</strong> determined that provide care and nursing activities<br />

(Table 3). Nursing assistant is a lower educational level of the nursing profession<br />

(obtained after two years of professional nursing education); this qualification level is<br />

fading out. Two nursing specialties are recognized by NIHDI for nurses of both<br />

qualification levels after completion of a post-graduate 40 hours-course: specialist nurse<br />

in diabetes and specialist nurse in wound care.<br />

In 2007, pilot projects were launched on the employm<strong>en</strong>t of care assistants in home<br />

nursing. Care assistants are allowed to perform a limited list of nursing interv<strong>en</strong>tions<br />

which are delegated by a nurse and under supervision of a nurse (Royal Decree 12<br />

January 2006, published on 3 February 2006). There are explicit requirem<strong>en</strong>ts for the<br />

qualification and recognition as a care assistant (Ministerial Circular <strong>Le</strong>tter of 8<br />

November 2006, published on 14 December 2006). They can only be employed by<br />

services for home nursing which are eligible to receive financing for specific costs of<br />

services for home nursing.<br />
