2.5.3 Curr<strong>en</strong>t situation<br />

Curr<strong>en</strong>tly, four paym<strong>en</strong>t systems and one special arrangem<strong>en</strong>t contribute to the<br />

financing of home nursing at the federal level:<br />

1. A mixed system of fee-for-service paym<strong>en</strong>t and lump sum paym<strong>en</strong>t: the<br />

nom<strong>en</strong>clature of nursing interv<strong>en</strong>tions and activities;<br />

2. Specific costs for home nursing organisations (since 2002);<br />

3. Subsidy for the costs related to computerization;<br />

4. Reduced social tax contributions (Social Maribel) and Social Agreem<strong>en</strong>ts<br />

betwe<strong>en</strong> the federal governm<strong>en</strong>t, organisations of employers and<br />

organisations of employees.<br />

2.5.3.1 The Nom<strong>en</strong>clature of Nursing Interv<strong>en</strong>tions and Activities<br />

Two types of nursing interv<strong>en</strong>tions and care provisions have to be distinguished: a feefor-service<br />

system for technical nursing interv<strong>en</strong>tions and a lump sum system for<br />

nursing interv<strong>en</strong>tions for pati<strong>en</strong>ts suffering from dep<strong>en</strong>d<strong>en</strong>cy/defici<strong>en</strong>cies in the activities<br />

of daily living (ADL) 20 .<br />

The nom<strong>en</strong>clature of home nursing summarizes a limited list of nursing care<br />

activities/interv<strong>en</strong>tions which are reimbursed by NIHDI (app<strong>en</strong>dix 3).<br />

The rules and tariffs for financing home nursing are <strong>en</strong>dorsed by the agreem<strong>en</strong>t<br />

committee. In the agreem<strong>en</strong>t committee, repres<strong>en</strong>tatives of home nurses on the one<br />

hand and repres<strong>en</strong>tatives of health insurance organisations on the other hand negotiate<br />

which nursing interv<strong>en</strong>tions are financed and they determine the tariffs of the<br />

interv<strong>en</strong>tions. The Medical Evaluation and Inspection Departm<strong>en</strong>t of the NIHDI is<br />

responsible to control care delivery.<br />

ADL-measurem<strong>en</strong>t<br />

The distinction betwe<strong>en</strong> fee-for-service financing and lump sum financing are based on<br />

the scores on the Belgian Evaluation Scale for Activities of Daily Living (BESADL).<br />

BESADL is an adaptation of the ‘Index of ADL’ 21 . The tool evaluates the six original<br />

domains of the ‘Index of ADL’: bathing (personal hygi<strong>en</strong>e), dressing, transfer, toileting,<br />

contin<strong>en</strong>ce, and eating (feeding). Each function is scored 1 (no help) to 4 (complete<br />

help), a higher score indicating higher dep<strong>en</strong>d<strong>en</strong>cy. Inter-rater accuracy of the<br />

dep<strong>en</strong>d<strong>en</strong>cy evaluations is under continuous supervision of the medical and nurse<br />

advisors of NIHDI and health insurance ag<strong>en</strong>cies.<br />

A pati<strong>en</strong>t is considered as dep<strong>en</strong>d<strong>en</strong>t for a function if the score for that function is<br />

higher than 2. Using a Boolean logic algorithm a global score is classified in one of four<br />

hierarchical dep<strong>en</strong>d<strong>en</strong>cy levels: baseline level (no dep<strong>en</strong>d<strong>en</strong>cy or low dep<strong>en</strong>d<strong>en</strong>cy level);<br />

level A (dep<strong>en</strong>d<strong>en</strong>cy for bathing, dressing and transfer or toileting); level B (dep<strong>en</strong>d<strong>en</strong>cy<br />

for bathing, dressing, transfer, toileting and contin<strong>en</strong>ce or eating); level C (the highest<br />

score for at least five functions, except contin<strong>en</strong>ce or eating, which may have a score 3)<br />

(Table 5).
