The scoring is embedded in a list of hierarchical mapping criteria, which makes that<br />

theoretical combinations are in some cases mapped in a higher level (for details see<br />

Arnaert (1999) 22 .<br />

Home care pati<strong>en</strong>ts reimbursed under at least level A dep<strong>en</strong>d<strong>en</strong>cy require daily hygi<strong>en</strong>ic<br />

care. A minimal dep<strong>en</strong>d<strong>en</strong>cy respectively id<strong>en</strong>tified by BESADL-scores or a doctor’s<br />

attestation confirming moderate to severe dem<strong>en</strong>tia, determines the subject’s eligibility<br />

for daily hygi<strong>en</strong>ic nursing care.<br />

Financing and BESADL<br />

Globally, 64 billing co<strong>des</strong> can be classified in various categories (see app<strong>en</strong>dix 2). The<br />

same billing co<strong>des</strong> apply for employed or self-employed home nurses.<br />

• Low dep<strong>en</strong>d<strong>en</strong>t pati<strong>en</strong>ts are reimbursed through fee-for-service related<br />

paym<strong>en</strong>ts. With exception of hygi<strong>en</strong>ic nursing care, a doctor's prescription is<br />

required for reimbursem<strong>en</strong>t of all nursing interv<strong>en</strong>tions in the fee-for-service<br />

paym<strong>en</strong>t system.<br />

• Pati<strong>en</strong>ts who score highest on care dep<strong>en</strong>d<strong>en</strong>cy (BESADL measurem<strong>en</strong>ts) are<br />

reimbursed through per diem lump sums. The lump sum system is a type of<br />

fee-for-service paym<strong>en</strong>t system based on the number of days of care 20 . For<br />

pati<strong>en</strong>ts with at least level A dep<strong>en</strong>d<strong>en</strong>cy, all nursing interv<strong>en</strong>tions are<br />

reimbursed by a lump sum per day, which covers all nursing care delivery on<br />

one day. Paym<strong>en</strong>ts are calculated on the need for nursing care during 24<br />

hours per day. A doctor's prescription is not required for reimbursem<strong>en</strong>t of<br />

nursing care delivery under the lump sum system, except for technical<br />

interv<strong>en</strong>tions under fee-for-service such as injections, wound care, bladder<br />

care, gastro-intestinal care, specific technical nursing interv<strong>en</strong>tions, …).<br />

• Additional per diem lump sums apply to palliative care and diabetic pati<strong>en</strong>ts.<br />

Over half of the overall amount (407 million euro) is reimbursed for per diem care in<br />

pati<strong>en</strong>ts for whom the degree of dep<strong>en</strong>d<strong>en</strong>cy is established by means of the BESADL<br />

scale. In 2003, 18 511 healthcare professionals delivered reimbursed homecare with an<br />

average reimbursed amount of 34 483€ per professional.<br />

Pseudo co<strong>des</strong> are established in order to docum<strong>en</strong>t the nursing interv<strong>en</strong>tions in pati<strong>en</strong>ts<br />

under the lump sum paym<strong>en</strong>t system. These pseudo co<strong>des</strong> have to be transmitted along<br />

with the nom<strong>en</strong>clature co<strong>des</strong> of the lump sum paym<strong>en</strong>ts.
