4.2.1.2 The Aide <strong>à</strong> la Personne âgée –APA scale<br />

Description<br />

This scale is used in Belgium for the granting of an allowance for elderly and disabled<br />

people.<br />

The APA scale comprised six items: moving, nutrition, hygi<strong>en</strong>e, house holding,<br />

communication, need of supervision. Each item is scored 0 (no problem), 1 (light<br />

problems), 2 (important problems) and 3 (impossibility). The sum of the scores gives<br />

eligibility for differ<strong>en</strong>t levels of allowance. If this score is lower than 7, there is no<br />

b<strong>en</strong>efit allowance. Score from 7 up to 8 gets category allowance I, from 9 up to 11,<br />

category allowance II, from 12 up to 14, allowance category III, from 15 up to 16<br />

category allowance IV and more th<strong>en</strong> 16, category allowance V.<br />

Validity<br />

The cont<strong>en</strong>t validity 40 shows that the scale assesses incapacities and does not meet the<br />

ICF approach. Some items measure incapacities for ADL, other ones for IADL.<br />

Moreover some items are mix assessm<strong>en</strong>t of ADL and IADL. The items are quite wide<br />

and heterog<strong>en</strong>eous, assessing differ<strong>en</strong>t dim<strong>en</strong>sions in one item. For example, the item<br />

concerning eating measures the ability of a pati<strong>en</strong>t to make his shopping and his cooking<br />

(which are IADL) and in the same time the capacity to swallow the food (which is an<br />

ADL). This scale does not assess incontin<strong>en</strong>ce disorders. The scale is well correlated<br />

with the Belgian scale (Spearman’s correlation coeffici<strong>en</strong>t > 0.7) and the AGGIR scale<br />

(Spearman’s correlation coeffici<strong>en</strong>t > 0.7). Cronbach alpha coeffici<strong>en</strong>t is higher than 0.8.<br />

External validity is low (kappa coeffici<strong>en</strong>t
