It would not be a good choice (as it was made in the Netherlands) to allow a third<br />

external party to perform the assessm<strong>en</strong>t and check the pati<strong>en</strong>t’s eligibility for nursing<br />

care.<br />

According to the stakeholders, the curr<strong>en</strong>t financing system offers little inc<strong>en</strong>tives for<br />

prev<strong>en</strong>tion and rehabilitation nursing and restorative nursing care.<br />

5.2.4.3 Topic 19: Comm<strong>en</strong>ts on the assessm<strong>en</strong>t tools<br />

BESADL tool<br />

On the one hand there were negative critiques with regard to the limited validity and<br />

reliability of the curr<strong>en</strong>t ADL evaluation scale in use. The BESADL tool only takes<br />

physical dep<strong>en</strong>d<strong>en</strong>cy into account, not social or other sources of need for nursing care.<br />

Persons characterized by the same level of physical dep<strong>en</strong>d<strong>en</strong>cy may repres<strong>en</strong>t very<br />

differ<strong>en</strong>t workloads for home nurses. The ADL instrum<strong>en</strong>t is not adapted to assess the<br />

physical dep<strong>en</strong>d<strong>en</strong>cy of childr<strong>en</strong>. The use of the instrum<strong>en</strong>t does not take suffici<strong>en</strong>tly<br />

into account a correction for the pres<strong>en</strong>ce of assistive devices.<br />

On the other hand the merits of this ADL assessm<strong>en</strong>t instrum<strong>en</strong>t were highlighted. The<br />

instrum<strong>en</strong>t is not perfect but it demonstrated validity in differ<strong>en</strong>tiating groups of<br />

pati<strong>en</strong>ts according to their need for nursing care. It is a simple instrum<strong>en</strong>t that is known<br />

by all nurses, it requires little time sp<strong>en</strong>ding and resources.<br />

Alternatives<br />

As m<strong>en</strong>tioned before, propositions for an alternative for the Belgian adapted KATZ<br />

assessm<strong>en</strong>t instrum<strong>en</strong>t varied widely. Betwe<strong>en</strong> all the propositions, some stakeholders<br />

expressed their prefer<strong>en</strong>ce for the RAI assessm<strong>en</strong>t instrum<strong>en</strong>t for home care (RAI-HC).<br />

Advantages of RAI were emphasized: it is se<strong>en</strong> as a complete and adequate assessm<strong>en</strong>t<br />

and it offers an integrated approach <strong>en</strong>abling to serve several purposes simultaneously:<br />

quality indicators, financing, support for plan of care, outcome measurem<strong>en</strong>t, etc ...<br />

However, the RAI instrum<strong>en</strong>t is considered to be too complex, too difficult, to require<br />

too much administrative efforts by home nurses. A new instrum<strong>en</strong>t should be simple,<br />

easy to understand and interpret.<br />

Participants also requested a shorter scre<strong>en</strong>ing instrum<strong>en</strong>t for detecting those pati<strong>en</strong>ts<br />

who need a full RAI-assessm<strong>en</strong>t. It was also m<strong>en</strong>tioned, both by home nurses and by<br />

repres<strong>en</strong>tatives of sickness funds, that if RAI is used only for those pati<strong>en</strong>ts requiring<br />

complex nursing care and/or multidisciplinary consultation, the time and effort which<br />

will be required for carrying out the assessm<strong>en</strong>t should be reimbursed.<br />

Key points<br />

• Registration and assessm<strong>en</strong>t tools were considered as an ess<strong>en</strong>tial part of<br />

nurse’s role in home care.<br />

• Stakeholders did not agree on the objectives of the use of an assessm<strong>en</strong>t<br />

instrum<strong>en</strong>t: for control of care delivery, case-mix determination and financing,<br />

quality indicators, care planning, …<br />

• Stakeholders reflected on the administrative burd<strong>en</strong> of a compreh<strong>en</strong>sive<br />

assessm<strong>en</strong>t: some considered that such a data collection and information<br />

gathering was necessary and feasible, others pleaded for a simple instrum<strong>en</strong>t.<br />

Some stakeholders ev<strong>en</strong> suggested to develop a new instrum<strong>en</strong>t, specific for<br />

home nursing in Belgium.
