Ancillary Pages
• Identify each appendix with letters if more than one (Appendix A, Appendix B, etc.),
and continue report pagination.
• Prepare a fly page for each appendix. A fly page should include the identifying letter
and the specific title for the content. For example:
Appendix A:
Ohio University Recycling Program Survey
• Refer to each appendix in the body of the paper.
• Put the appendices in the order in which they are introduced in the body of the paper.
References
• Use Chicago Style format for endnotes and Bibliography.
• Alphabetize Bibliography.
