III. Household Survey Questionnaire<br />
Page 1 Location and responsible people<br />
The first page of the questionnaire provides the basic background information on the survey location<br />
and date (we need all of this information!), and those people that were involved in, and responsible<br />
for, the accuracy of the questionnaire. The respondent should be the head of household and his/her<br />
spouse together. This is important that we interview both of the spouse together.<br />
Write in the name of the Village, District, Upazilla, WFP Zone, code numbers and Respondent’s<br />
Name and Serial Number. The Supervisor will provide the codes for WFP Zone, District, Upazilla,<br />
Cluster, Village. For codes, enter one numeric digit in each box. For example, if the Zoba code is<br />
‘1’, the Supervisor will enter ‘01’. The household codes are 01-20 for each village, and will be<br />
determined by the Supervisor after the surveys are complete.<br />
The sex, marital status, and total number of household members are clear.<br />
Supervisors will sign the form after they have carefully checked the form for completeness,<br />
accuracy, and clarity. This is a critical step because the people who will do the data entry from the<br />
forms will not have any context to interpret missing or unclear information or cover for mistakes.<br />
A. Household members<br />
Please make sure, that the information of the household head are recorded on the first row.<br />
For the coded entries in the matrix, be sure to write the correct coded response in the box for each<br />
household member listed. For this matrix, do not circle the code.<br />
A1. ‘Respondent’s relationship’ is coded. Head of Household head = ‘1’, Husband wife = ‘2’, Son or<br />
daughter = ‘3’ and so on. For each household member row, write the correct code.<br />
A2. ‘Sex’ is coded Male = ‘1’ and Female = ‘2’. For each household member row, write the correct<br />
code.<br />
A3. Record ‘Age’ in number of years, for each household member. If less than 1 year write 0.<br />
A4. Write ‘1’ if the member is not married, ‘2’ for married, ‘3’ for Divorced, ‘4’ for Widowed, ‘5’<br />
for Separated.<br />
A5. Employment Status information is required for the household members whose age 15 years and<br />
above. The codes are provided on the following page.<br />
A6. Employment Stability refers to the stability of job. The codes are provided on page 3 of the<br />
questionnaire. Write the code in the correct box.<br />
A7. Primary Economic Activity refers to the primary occupational skills a person relies on as a<br />
livelihood strategy. For example a household member may primarily engage as agricultural labour<br />
