Voucher no: Each voucher should have a unique number <strong>for</strong> easy tracking the commodities.<br />
Issuing store: Name of the store from where the commodities are issued<br />
Ship to: Name of the facility which the commodities are dispatched<br />
Article no (A): Each commodity should be recorded <strong>on</strong> a separate line described by a number.<br />
Commodity name (B): Names of the issued commodities.<br />
Batch number (C): Batch/Lot number of issued commodities products.<br />
Expiry date (D): Date of expirati<strong>on</strong> as written <strong>on</strong> the product label<br />
Freeze indicator status(E) Freeze indicator readings in the issuing facility<br />
VVM status (F): Vaccine Vial M<strong>on</strong>itor readings in the issuing facility.<br />
Amount (G): Number of commodities issued (in doses <strong>for</strong> vaccines and diluents, pieces <strong>for</strong> other items).<br />
Freeze indicator status(H) Freeze indicator readings in the receiving facility.<br />
VVM status (I): Vaccine Vial M<strong>on</strong>itor readings in the receiving facility.<br />
Amount (J): Number of commodities issued.<br />
Remarks (K): Explanati<strong>on</strong> of differences between issues and receipts - if any.<br />
Approved by/date: Name and signature of the authority approving this shipment and approval date (dd/mm/yyyy)<br />
Shipped by/date: Name and signature of the authority that shipped the commodities and shipping date (dd/mm/yyyy)<br />
