Directions for Changing the Name of a Career School and College<br />

Appendix 15<br />

A. Submit an original and one copy of the following:<br />

1. PS-001 (R/C/S) Application for Certificate of Approval using the new name of<br />

the school.<br />

2. PS-001W Owner’s Affidavit for each owner listed on the application, using the<br />

new name of the school.<br />

3. PS-001V Affidavit of On-Campus Enrollment using the new name of the<br />

school. (Not required for seminar only or correspondence schools or residence<br />

schools with a Detachable Notice of Cancellation on enrollment agreement.) ***<br />

4. PS-001Z Director’s Statement using the new name of the school.<br />

5. School Catalog or "Seminar Only School Conveyance Requirements" document -<br />

see Catalog <strong>Guide</strong> PS-001X*** or Seminar Only School Conveyance<br />

Requirements enclosed, using the new name of the school where applicable.<br />

(Addendum and paste-over may be used until next printing). All Catalog changes<br />

for residence and correspondence schools shall be accompanied by the form PS-<br />

042R Summary of Changes. ***<br />

6. Enrollment Agreement, see Enrollment Contract Checklist (PS-190) enclosed,<br />

using the new name of the school, with the Summary of Changes PS-042.<br />

(Enrollment agreement or supplements not required for seminar only school.) ***<br />

7. All other documents (and advertisements) where the name of the school appears,<br />

using the new name of the school.<br />

B. Submit the fee of $150 for the school and $15 for each representative.<br />

C. Submit evidence of registration of the new assumed name with the county clerk.<br />

Evidence of registration with the Secretary of State is also required if the owner is a<br />

corporation.<br />

D. Submit a rider to the bond to show as principal the owner dba the new name of the<br />

school.<br />

NOTE: Certain types of schools are prohibited from using the word “college” or “university.” Should<br />

you wish to use either of these words in the school or corporate name, you should first obtain approval<br />

from the <strong>Texas</strong> Higher Education Coordinating Board, 7745 Chevy Chase Drive, Austin, <strong>Texas</strong> 78752, and<br />

provide a copy of that approval with your new application.<br />
