Schools must periodically renew Certificates of Approval.<br />

The requirements vary for different schools, and this chapter<br />

discusses each.<br />

CSC will thoroughly review a renewal application submitted<br />

by the career school or college. If CSC determines the<br />

application is complete and the school is in compliance,<br />

CSC will issue a Certificate of Approval.<br />

Large schools have annual gross revenue exceeding<br />

$100,000.00 generated from tuition and fees minus refunds,<br />

or schools receiving Title IV funds. The Law requires these<br />

schools to renew their Certificates of Approval annually.<br />

CSC staff will mail a renewal application letter and packet to<br />

the school director (see Appendix 34).<br />

To renew the Certificate of Approval, the school must<br />

submit the following documentation:<br />

• form PS-015, Application for Renewal of Certificate of<br />

Approval (see Appendix11)<br />

• form PS-186, Fee Sheet with appropriate fees<br />

amount of renewal fee (based on .31% of the tuition<br />

and fees minus refunds as indicated on the school’s<br />

most recent financial statement, or $500.00,<br />

whichever is greater)<br />

form PS-014A, Addendum to Representative<br />

Application with required fees (see Appendix 10)<br />
