The instructions for form PS-072, Section D, are on page 6<br />

of the PS-072. A school may use this form or a substitute<br />

spreadsheet that contains all of the same fields. Each field<br />

must be completed.<br />

Seminar schools require much less information than<br />

residence schools.<br />

Seminar schools need to return only page 2 of the PS-073<br />

(see Appendix 22). The school must submit this no later<br />

than December first of each year.<br />

CSC requires only the following two items for seminar<br />

schools:<br />

1. The total annual number of students enrolled during the<br />

reporting period:<br />

includes all enrolled students who attended seminars<br />

during the reporting period.<br />

includes all students who dropped out before<br />

completing seminar(s), those who completed<br />

seminar(s), and those who were still attending<br />

seminar(s) at the end of the reporting period.<br />

Note: If students attend more than one<br />

seminar, count each seminar they attend<br />

during the reporting period. For example, if a<br />

student attended three seminars at a school<br />

during the reporting period, the school counts<br />

the student’s attendance three times.<br />
