Seminar School Seminars<br />

Required Items<br />

for New Seminar<br />

Applications<br />

New seminar submissions a seminar school will offer<br />

require the following:<br />

• form PS-304S, New/Revised Seminar Application (see<br />

Appendix 31)<br />

• course handout<br />

• the original and one copy of form PS-042SC, Summary<br />

of Changes (see Appendix 19)<br />

• form PS-186, Fee Sheet (see Appendix 26)<br />

• $35.00 fee for each new seminar submission.<br />

Note: For seminars longer than three consecutive<br />

calendar days, schools are required to enroll the<br />

student with an enrollment agreement (see Appendix<br />

27, Form PS-190, Enrollment Contract Checklist).<br />

Page 1 of form PS-304S must include the:<br />

• seminar title<br />

• seminar objectives<br />

• seminar admissions requirements<br />

• seminar length (provide in clock hours, not days; account<br />

for break time)<br />

• seminar tuition and fees (provide detail of fees on page 2,<br />

and include book titles)<br />

• method used to pay tuition/fees.<br />
