Chapter 4: The voices of older persons<br />

“They have become more economical with <strong>the</strong> level of care.<br />

To beg<strong>in</strong> with, it worked out very well for us but now <strong>the</strong>y<br />

are very conscious about <strong>the</strong>ir money.” rural Germany<br />

“The health card was <strong>in</strong>troduced <strong>in</strong> 2002 and <strong>the</strong>n <strong>in</strong> 2008<br />

it was withdrawn for all people over a certa<strong>in</strong> <strong>in</strong>come.<br />

It used to be for everybody over 70 regardless of <strong>in</strong>come.<br />

Now <strong>the</strong>re is also a charge for prescriptions.” urban Ireland<br />

Lack of attention to age-related health issues<br />

A few participants highlight a lack of medic<strong>in</strong>es to<br />

treat age-related diseases and of facilities to diagnose<br />

or treat <strong>the</strong>m.<br />

“Medic<strong>in</strong>es are available <strong>in</strong> <strong>the</strong> health centre but for older<br />

people <strong>the</strong>se are usually pa<strong>in</strong>killers, antibiotics or malaria<br />

medic<strong>in</strong>e. You will not f<strong>in</strong>d medic<strong>in</strong>e for diabetes,<br />

hypertension or o<strong>the</strong>r age-related diseases.” urban Uganda<br />

“The local health centres or hospitals do not have <strong>the</strong><br />

facilities to measure my blood sugar. They cannot even<br />

advise me what to do.” rural Bangladesh<br />

The attitudes of health professionals are often<br />

experienced as a barrier to receiv<strong>in</strong>g appropriate health<br />

care. Age discrim<strong>in</strong>ation, neglect and a lack of tra<strong>in</strong><strong>in</strong>g<br />

<strong>in</strong> age-related diseases lead<strong>in</strong>g to weak diagnosis are<br />

<strong>the</strong> most frequently identified problems with health<br />

professionals, both <strong>in</strong> hospitals and <strong>in</strong> health centres.<br />

This is not reported <strong>in</strong> Nor<strong>the</strong>rn and Western Europe<br />

or Canada.<br />

“Hospitals are not very helpful to you when you are old;<br />

<strong>the</strong>y treat you as if you are a burden.” rural Nigeria<br />

“In <strong>the</strong> government hospital <strong>the</strong>re is a separate counter<br />

for older people but it is not function<strong>in</strong>g and always rema<strong>in</strong>s<br />

closed. Older people suffer <strong>in</strong> <strong>the</strong> long queue to get<br />

medic<strong>in</strong>es…. Even though <strong>the</strong>re is a system, <strong>the</strong> benefit<br />

does not accrue to <strong>the</strong> older people because of neglect.”<br />

“My wife has many diseases; I often take her to <strong>the</strong><br />

hospital. She has got t<strong>in</strong>nitus now. When we came to see<br />

<strong>the</strong> doctor <strong>in</strong> <strong>the</strong> hospital this doctor said, ‘Your disease is<br />

<strong>the</strong> old-age disease, it is <strong>in</strong>curable.’ This is irresponsible.”<br />

urban Viet Nam<br />

“The serious problem for me is with ambulances. I call<br />

<strong>the</strong>m when I feel bad. They first ask me how old I am. If I say<br />

I am over 70 <strong>the</strong>y say ‘We cannot send an ambulance, you<br />

had better treat yourself’.” rural Kyrgyzstan<br />

“We have no medical experts who specialize <strong>in</strong> older<br />

people. Nowadays one doctor treats everybody <strong>in</strong> <strong>the</strong> same<br />

way, or does not treat us at all. You go to <strong>the</strong>m already<br />

know<strong>in</strong>g that <strong>the</strong>y will not help you.” urban Bosnia and<br />

Herzegov<strong>in</strong>a<br />

However, some participants gave more positive<br />

examples.<br />

“Recently, <strong>the</strong> doctor expla<strong>in</strong>ed to me some th<strong>in</strong>gs about<br />

my heart; he expla<strong>in</strong>ed how to take <strong>the</strong> drugs. It seems to<br />

me that he knows what he is do<strong>in</strong>g.” urban Peru<br />

Especially <strong>in</strong> Asia and <strong>the</strong> Pacific, older people report<br />

that <strong>the</strong>y are generally well treated due to <strong>the</strong> culture<br />

of respect for older people.<br />

“The staff members at health centres are very good.<br />

They know how to respect us and <strong>the</strong>y do not make us wait<br />

for too long when we are sick.” peri-urban Cambodia<br />

Mental health issues<br />

The consultations <strong>in</strong>dicate that mental health care for<br />

older people is not receiv<strong>in</strong>g sufficient attention, except<br />

<strong>in</strong> developed countries. Participants report an overall<br />

lack of special services for mental illnesses and even a<br />

worsen<strong>in</strong>g situation of care.<br />

“In <strong>the</strong> case of a mental health issue we know that a person<br />

has to go to <strong>the</strong> hospital but when we go to <strong>the</strong> hospital<br />

<strong>the</strong>y do not register <strong>the</strong> cases of older people.” urban<br />

Mozambique<br />

“There is no special treatment or care for older people with<br />

mental health issues.” rural Cambodia<br />

“People who are more dependent or who have more serious<br />

physical or mental health issues have more difficulties.<br />

For <strong>the</strong>m <strong>the</strong>re has been no improvement, because <strong>the</strong>re is<br />

no social care support.” Paraguay<br />

In some developed countries, participants <strong>in</strong>dicate that<br />

<strong>the</strong>re is a change for <strong>the</strong> better <strong>in</strong> <strong>the</strong> treatment of those<br />

who have a mental illness.<br />
