concern <strong>in</strong> developed countries, while social protection<br />
and old-age pension coverage rema<strong>in</strong> a challenge for<br />
develop<strong>in</strong>g countries, where a large proportion of <strong>the</strong><br />
labour force is found <strong>in</strong> <strong>the</strong> <strong>in</strong>formal sector. While<br />
private transfers from family members were typically<br />
expected to provide a significant source of <strong>in</strong>come for<br />
older persons, chang<strong>in</strong>g family structures may result<br />
<strong>in</strong> lower levels of support <strong>in</strong> <strong>the</strong> future.<br />
Accord<strong>in</strong>g to some researchers, <strong>the</strong>re is no evidence<br />
that population age<strong>in</strong>g has underm<strong>in</strong>ed economic<br />
development and that countries do not have sufficient<br />
resources to ensure pensions and health care for an<br />
older population. Never<strong>the</strong>less, globally, only one third<br />
of countries have comprehensive social protection<br />
schemes, most of which only cover those <strong>in</strong> formal<br />
employment, or less than half of <strong>the</strong> economically<br />
active population worldwide. While social pensions<br />
are an important end <strong>in</strong> <strong>the</strong>mselves s<strong>in</strong>ce <strong>the</strong>y make<br />
a big difference <strong>in</strong> <strong>the</strong> well-be<strong>in</strong>g of older persons,<br />
<strong>the</strong>y have also been shown to benefit entire families.<br />
The report po<strong>in</strong>ts to <strong>the</strong> need for policy change to build<br />
stable protection schemes for an <strong>in</strong>creas<strong>in</strong>g number<br />
of older persons. Questions of solidarity, accessibility,<br />
affordability and susta<strong>in</strong>ability will need to be<br />
addressed when consider<strong>in</strong>g <strong>the</strong> options available to<br />
ensure a m<strong>in</strong>imum level of protection for all people<br />
after a certa<strong>in</strong> age.<br />
The need to ensure access to<br />
quality health care<br />
In order to realize <strong>the</strong>ir right to enjoy <strong>the</strong> highest<br />
atta<strong>in</strong>able standard of physical and mental health,<br />
older persons must have access to affordable health care<br />
and services that meet <strong>the</strong>ir needs. This <strong>in</strong>cludes<br />
preventive and curative care and rehabilitation.<br />
A lifecourse perspective should <strong>in</strong>clude health<br />
promotion and disease prevention activities that focus<br />
on ma<strong>in</strong>ta<strong>in</strong><strong>in</strong>g <strong>in</strong>dependence, prevent<strong>in</strong>g and delay<strong>in</strong>g<br />
disease and disability, and provid<strong>in</strong>g treatment.<br />
Policies are needed to promote lifelong health, assistive<br />
technology, rehabilitative care, promotion of healthy<br />
lifestyles, and supportive environments. Tra<strong>in</strong><strong>in</strong>g of<br />
caregivers and health professionals is essential to<br />
ensure that those who work with older persons have<br />
access to <strong>in</strong>formation and basic tra<strong>in</strong><strong>in</strong>g <strong>in</strong> <strong>the</strong> care<br />
of older people.<br />
The report po<strong>in</strong>ts out that good health must lie at <strong>the</strong><br />
core of society’s response to population age<strong>in</strong>g.<br />
Ensur<strong>in</strong>g that people live healthier as well as longer<br />
lives will result <strong>in</strong> greater opportunities and lower costs<br />
to older persons, <strong>the</strong>ir families and society. The report<br />
describes a four-po<strong>in</strong>t strategy to foster healthy and<br />
active age<strong>in</strong>g throughout <strong>the</strong> lifecourse that focuses on<br />
promot<strong>in</strong>g healthy behaviours at all ages, early detection<br />
Campaign<strong>in</strong>g for a pension <strong>in</strong> Paraguay.<br />
and quality care to m<strong>in</strong>imize <strong>the</strong> consequences of<br />
chronic disease, creat<strong>in</strong>g physical and social environments<br />
that foster <strong>the</strong> health and participation of older persons,<br />
and chang<strong>in</strong>g social attitudes to encourage <strong>the</strong>ir<br />
participation.<br />
The need for budget allocation to<br />
develop, implement and evaluate<br />
evidence-based policies<br />
While <strong>the</strong>re has been substantial progress <strong>in</strong> adopt<strong>in</strong>g<br />
new policies and legislation on age<strong>in</strong>g and ma<strong>in</strong>stream<strong>in</strong>g<br />
age<strong>in</strong>g <strong>in</strong>to exist<strong>in</strong>g legal and policy frameworks, <strong>the</strong><br />
evidence suggests that implementation and budget<br />
allocation is still <strong>in</strong>complete. Accountable budgets are<br />
necessary to support policies. There is also a need for<br />
capacity development <strong>in</strong> government, academic and<br />
voluntary-sector <strong>in</strong>stitutions to enable <strong>the</strong> formulation,<br />
implementation and monitor<strong>in</strong>g of policies and<br />
<strong>in</strong>terventions. Concrete, accountable, gender-sensitive<br />
and funded actions to ensure key commitments of <strong>the</strong><br />
Madrid Plan are still needed across all its key policy<br />
doma<strong>in</strong>s. <strong>Age<strong>in</strong>g</strong> must be ma<strong>in</strong>streamed across all policy<br />
sectors and action on age<strong>in</strong>g can no longer be subord<strong>in</strong>ate<br />
to o<strong>the</strong>r issues.<br />
Contributors to this report agree that <strong>the</strong>re must be<br />
improvements <strong>in</strong> data collection and analysis as well<br />
as more research to ensure effective monitor<strong>in</strong>g of <strong>the</strong><br />
Madrid Plan. The evidence shows that <strong>the</strong> absence of<br />
national and sub-national data on poverty, health,<br />
<strong>in</strong>clusion and contributions of older people may be<br />
systematically exclud<strong>in</strong>g <strong>the</strong>m from development plans<br />
and public policy provision.<br />
